Isotretinoin (Generic Accutane)

Accutane is a form of vitamin A that is used in treatment of severe acne. Normally, it is only given if other options, including antibiotics, have been exhausted. The generic form of Accutane is called isotretinoin. It reduces how much oil your skin produces and increases the rate of cellular turnover.

Perhaps the most important thing patients considering isotretinoin should be aware of is that it can cause very severe birth defects that can be life threatening if a pregnant woman ingests it. Even one dose of isotretinoin can cause major birth defects of the eyes, ears, skull, face, heart, and brain.

The risk of defects is so high and so potentially severe that women who are of childbearing age have to agree to use two specified forms of birth control and to have regular pregnancy tests before, during, and after taking isotretinoin. To be considered non-childbearing, a woman must have had a total hysterectomy, or have been in menopause for at least one year. This means that even women who have had their tubes tied are considered “childbearing” unless they have been in menopause for 12 months.

Isotretinoin users must inform their doctors if they have a personal or family history of mental illness or depression, high cholesterol, heart disease, or high triglyceride levels, bone disorders such as osteoporosis, diabetes, asthma, anorexia or other eating disorder, or liver disease.

Isotretinoin is to be taken exactly as prescribed with a full glass of water. If the capsule melts in your esophagus, it can cause irritation. Do not chew the capsule, but swallow it whole. Isotretinoin is to be taken with water or milk and food. Acne may worsen at first, but should then improve. Isotretinoin should not be shared, even with someone who has the same symptoms.

You should not take vitamin A supplements when on isotretinoin. Isotretinoin can weaken bones, so it is important to avoid contact sports or other risky activities. You should not donate blood while taking isotretinoin, or for at least one month after you stop taking it. You should not use wax hair removers or have laser skin treatments or microdermabrasion while you are taking isotretinoin because scarring may result. Isotretinoin makes you more sensitive to UV rays from the sun or from tanning beds. It can also impair your night vision.

If you experience any of the following side effects, call your doctor immediately:

Hives, rash, difficulty breathing, facial swelling, or swelling of the lips, tongue, and throat

Depression, sleep problems, crying spells, aggression, or changes in behavior

Numbness or weakness that comes on suddenly

Blurred vision, severe headache or pain behind the eyes, with or without vomiting

Loss of hearing or ringing in the ears

Seizures or severe upper stomach pain or pain of the neck, back or chest, with or without vomiting

Jaundice or yellowing of the eyeballs

Diarrhea, rectal bleeding, or dark colored urine

Body aches, flulike symptoms, easy bruising or bleeding, bone or joint pain

Other less serious side effects may include contact lens irritation, mild joint or back pain, dizziness, drowsiness, or nervousness, peeling skin or changes in fingernails or toenails.
