Introduction
Naratriptan is a serotonin 5-HT1 receptor agonist that reduces the substances which are responsible for headache pain, nausea, sensitivity to light and sound and other migraine symptoms. But it does not prevent headaches or reduce the number of attacks. Naratriptan works by narrowing blood vessels in the brain. Carrying the brand name of Generic Amerge , it comes as tablets to be taken by mouth. Each Generic Amerge tablet contains croscarmellose sodium, hypromellose, lactose, magnesium stearate, microcrystalline cellulose, triacetin, titanium dioxide, iron oxide along with naratriptan hydrochloride.
Indication
The Generic Amerge tablets are indicated for treating migraine attacks with or without aura and management of hemiplegic or basilar migraine, both of which are common with adults.
Mechanism of Action
Being a serotonin 5-HT1 receptor agonist, the Generic Amerge tablets lead to the activation of 5-HT1D/1B receptors resulting in vasoconstriction which is correlated with the relief of migraine headache. These 5-HT1D/1B receptors are located on intracranial blood vessels and at the arteriovenous anastomoses. In some cases, the 5-HT1D/1B receptors help inhibition of pro-inflammatory neuropeptide release by acting on the sensory nerves and helping in curing head aches.
Dosage and Administration
The dosage of Generic Amerge tablets varies from person to person, so strict adherence to the direction of your doctor is needed. The normal dosage of Naratriptan hydrochloride is 2.5 mg or one tablet. The dose may be repeated once at an interval of four hours but the patient should not be given a dose of 5 mg in a period of 24 hours and the continuation with these tablets should not be made for more than a month.
Patients with severe renal impairment and severe hepatic impairment should not be administered with this drug and the patients with mild to moderate renal impairment and mild to moderate hepatic impairment should not be given a dose more than 2.5 mg per day. They may be considered for a low starting dose.
Side Effects
Commonly seen side effects of Naratriptan tablets are paresthesias, dizziness, drowsiness, malaise/fatigue, and throat/neck pain. The symptoms of palpitations, increased blood pressure, tachyarrhythmias, and abnormal ECG, polydipsia, dehydration, and fluid retention, photophobia, hyposalivation and vomiting, bronchitis, cough, and pneumonia, muscle pain, arthralgia and articular rheumatism and vertigo are most severe but rarely seen side effects of these tablets.
The persistence of the side effects or development of any symptom other than stated above should me immediately reported to your doctor and adequate measures should be taken to get rid of them.
Drug Interaction
The Generic Amerge tablets are highly interactive with selective serotonin reuptake inhibitors/serotonin norepinephrine reuptake inhibitors and serotonin syndrome resulting in life threatening consequences. Thus their usage with this drug is contradicted.
Prolonged vasospastic reactions results from concomitant usage of ergot-containing drugs and Naratriptan tablets.
Precaution
- Patients who experience signs or symptoms suggestive of angina after taking Naratriptan should be evaluated for the presence of CAD or a predisposition to Prinzmetal. Proper measures for these should be provided.
- Patients with diseases that may alter the absorption, metabolism, or excretion of drugs, such as impaired renal or hepatic function should be carefully administered with this drug and close observation of their absorption, metabolism, or excretion of drugs should be made.
- Before treating headache in patients who were not previously diagnosed with migraine or who had never experienced a headache that is atypical for them, care should be taken to exclude other potentially serious neurological conditions.
- Before taking these medicines, you must let your doctor know if you are pregnant or have plans for that or if you are breast feeding.
- Your doctor should be informed about all other medications which you are taking along with these tablets.
Storage
These tablets should be stored at controlled room temperature of 20° to 25° C and 68° to 77° F. Keep them away from children and pets. Avoid its direct contact with heat, light and moisture.
