Introduction:
Phenytoin Sodium is a prescribed antileptic drug used for treatment of patients who are suffering from grand mal seizures and temporal lobe seizures. The grand mal seizure is characterized by a condition in which the patient evidences a sudden loss of consciousness followed by generalized convulsions in no time and temporal lobe seizure is caused by disease in the cortex of the temporal lobe of the brain which affects the senses of smell, taste, sight, hearing, memory, and movement. Under the brand name of Generic Dilantin, the Phenytoin Sodium is available in forms of capsules with strength of 100mg. Each capsule of Generic Dilantin contains colloidal silicon dioxide, hydroxyethyl cellulose, magnesium oxide, magnesium stearate, microcrystalline cellulose, povidone and sodium lauryl sulfate along with Phenytoin Sodium.
Indication:
The Generic Dilantin capsules are indicated for the control of generalized tonic-clonic grand mal and complex partial seizures. It also used for preventing and treating seizures occurring during or following neurosurgery.
Mechanism Of Action:
The motor cortex is the primary site of action where spread of seizure activity is inhibited. Phenytoin Sodium tends to stabilize the threshold against hyperexcitability which is caused by excessive stimulation or environmental changes capable of reducing membrane sodium gradient by promoting sodium efflux from neurons. It reduces the posttetanic potentiation at synapses which in return prevents cortical seizure foci from detonating adjacent cortical areas. The maximal activities of brain stem are responsible for the tonic phase of tonic-clonic (grand mal) seizures. The Generic Dilantin capsules reduce these activities.
Dosage And Administration:
An initial dose of 300-mg or three capsules per day may be given to patients who have received no previous treatment. The capsules should be taken three times in a day, one capsule of 100mg at each time.
Adults may be administered with a single dose of 300mg or three capsules at a time if, in them, seizure control has been established with divided doses of three 100-mg per day. But patients should be careful enough to not to miss the dose any day.
Patients may be administered a loading dose of 1 gm divided into three doses of 400 mg, 300 mg and 300 mg. After observing the frequent serum level, the normal maintenance dosage may be initiated.
The maximum daily dose for pediatrics should not exceed 300 mg per day and should be initially administered with a dosage of 5 mg/kg/day in two or three equally divided doses. They may be administered with a maintenance dosage is of 4 to 8 mg/kg.
Missed Dose
If you seem to miss a dose, take it immediately you remember. But you should not take double dose to make up the missed dose.
Overdose
If this drug is taken in excess, it can cause serious consequences. If you suppose to take an overdose of the drug, call your doctor without delay.
Side Effects:
The most common side effects of Generic Dilantin capsules are swollen glands, easy bruising or bleeding;, swollen or tender gums, fever, sore throat, and headache with a severe blistering, peeling, and red skin rash, first sign of any skin rash, no matter how mild, confusion, hallucinations, unusual thoughts or behavior, slurred speech, loss of balance or coordination, tremor (uncontrolled shaking), restless muscle movements in your eyes, tongue, jaw, or neck, extreme thirst or hunger, urinating more than usual, loss of appetite, dark urine, clay-colored stools, jaundice, changes in the shape of your face or lips.
Symptoms of mild skin rash or itching, dizziness, nervousness, sleep problems (insomnia),nausea, vomiting, constipation, headache, joint pain are less severe side effects of the drug.
Drug Interaction
Some drugs may interact with Dilantin. The patient must notify his health care professional if he is taking any other drugs, particularly any of the following: Alcohol, Diazepam (Valium®), Amiodarone (Cordarone®), Phenobarbital, Digoxin (Digitek®, Lanoxin®), Carbamazepine, Disulfiram (Antabuse®), Doxycycline, Calcium-containing products, Chlordiazepoxide (Librium®, Limbitrol®, Librax®), Cimetidine (Tagamet®), Corticosteroids, H2 blockers, Isoniazid, prednisone, and many others.
Contradiction:
The use of Phenytoin is contradicted in persons who have developed hypersensitivity to Phenytoin or other Hydantoins or any other component of the drug.
Storage:
These capsules should be stored in tight, light-resistant containers at 20-25oC (68-77oF). Protect these tablets from moisture.
