Introduction

Generic Myotonine (Bethanechol) 25 mg tablet is applied for treatment and prevention of bladder dysfunction caused by phenothiazines, non-obstructive urinary retention and retention caused by neurogenic bladder, gastroesophageal reflux, diagnosis of flaccid or atonic neurogenic bladder.

Mode of Action

Generic Myotonine (Bethanechol) stimulates cholinergic receptors in the smooth muscle of the urinary bladder and gastrointestinal tract. It leads to increased peristalsis, increased GI and pancreatic secretions, bladder muscle contraction which initiates urination. Apart from that it also increases ureteral peristaltic waves.

Dosage & administration

Usual dosage for children for treatment of abdominal distention or urinary retention is 0.6 mg/kg/day divided 3-4 times/day. For gastroesophageal reflux, it is 0.1-0.2 mg/kg/dose given 30 minutes to 1 hour before each meal to a maximum of 4 times/day. So far as S.C. is concerned, the usual dosage is 0.15-0.2 mg/kg/day divided 3-4 times/day.

For adults, 10-50 mg dosage 2-4 times per day is recommended. S.C. needs 2.5-5 mg dosage 3-4 times per day, up to 7.5-10 mg every 4 hours for neurogenic bladder.

Oral dosages must be taken 1 hour before meals or 2 hours after meals to keep away from nausea or vomiting. It may lead to abdominal discomfort, sweating, salivation, or flushing. Notify doctor if these symptoms become prominent; rise slowly from sitting/lying down. Be cautious when driving or climbing stairs.

Potential Adverse Effects

Symptoms of overdose include nausea, vomiting, abdominal cramps, diarrhea, involuntary defecation, flushed skin, hypotension, bronchospasm Atropine is the treatment of choice for intoxications manifesting with significant muscarinic symptoms; atropine I.V. 0.6 mg every 3-60 minutes (or 0.01 mg/kg I.V. every 2 hours if needed for children) should be repeated to control symptoms and then continued as needed for 1-2 days following the acute ingestion. Epinephrine 0.1-1 mg S.C. may be useful in reversing severe cardiovascular or pulmonary sequel.

Drug Contraindications

Bethanechol should not be administered to patients with hyperthyroidism, epilepsy, peptic ulcer disease, obstructive pulmonary disease, vasomotor instability, bradycardia, atrioventricular conduction defects, or Parkinsonism; hypotension, I.M. or I.V. use due to a likely severe cholinergic reaction. This drug should not be applied in people who have hypersensitivity to Bethanechol, mechanical obstruction of the GI or GU tract.

Warnings & Precautions

Apply caution when administering to nursing women, as it is unidentified if Myotonine is excreted in breast milk. The safety of this drug has not been studied in children less than 5 years of age. Syringe containing atropine should be readily available for treatment of serious side effects. Do not stop this medication without prior discussion with your doctor. Sustain adequate hydration unless instructed to restrict fluid intake. If you feel vomiting or loss of appetite, take small frequent meals and make frequent mouth care. Sucking lozenges or chewing gum may assist. It is essential to notify doctor if you are or wish to become pregnant.
